Camper Information & Forms:

If your child will require medication during the camp day, please print, complete and return this two-page form to the Camp Office by May 1. Please click here for the "Authorization to Medicate" form.

Please click here to request changes to your programs and/or transportation. Requests for changes after June 1 will be based on availability and space. Each change after June 1 will incur a $25 administrative fee.

Staff Information & Forms:

New staff applicants must provide at least two references. Your references should complete the Reference Questionnaire form and return it to Camp JCC as soon as possible. Applications will not be considered complete until this Reference Questionnaire is received. Returning Camp JCC staff do not need to submit this form.
