Now that you've registered for the After School Program and/or The Zone, we need you to complete the following required forms:

• Behavior Form
• Signature Release Form 
• Emergency Form (Electronic Version)
• Health Inventory (Electronic Version)
• Parents Guide to Regulated Child Care Form (requires parent/guardian signature only) 
• Additional Forms
  • Medication Authorization — if applicable (Electronic Version)
  • Family Situation Form — if applicable
